REGISTRATION BY MAIL
This mail-in application may be used, or use type-in form before printing, (hand write your email address-optional). YOU MUST PRINT, SIGN, AND MAIL if using one of these forms, and it must be postmarked by the close of registration for any election.
- Print with a black pen and complete all areas of the application. INCLUDE REQUIRED ID NUMBERS on the application or it will be rejected.
- It is OPTIONAL to give your phone number and/or email address. If given, it can help us communicate with you; HOWEVER by law it becomes part of your voter record which is made available to candidates and political parties (not to the general public.)
- Sign the application.
- Mail to Bloomington Election Commission, 115 E. Washington St., Room 403, PO Box 2400, Bloomington, IL 61702.

Voter registration in Illinois is not available by email, fax or telephone.
